US Cellular is unique—they require CDMA-less provisioning for some newer Androids (like newer Samsungs). Call *611 and ask them to "re-provision the SIM on the backend."
Unlike Verizon or T-Mobile, US Cellular relies heavily on a Device IMEI Whitelist . You cannot simply swap a SIM from an old USC phone into a random unlocked phone. The new phone’s IMEI must be in their database.
